Privacy Notice
This notice explains how the PERHORC demonstration prototype handles the self-reported information you enter into the questionnaire.
Nature of the information collected
The questionnaire may contain sensitive information about your health, lifestyle, diet, fitness, sleep, stress and behaviour. This information can be personal, and in some jurisdictions may be considered a special category of personal data.
How information is processed today
- For this prototype, responses are processed locally in your browser (via
localStorage) where applicable. - The generated strategy report is created client-side in your browser.
- You should only submit information you are comfortable entering.
- The platform does not intentionally collect emergency medical information for crisis response — always seek emergency help for urgent symptoms.
Downloaded PDF reports
The strategy report you download is a normal PDF file saved to your device. It may contain sensitive personal information. You are responsible for protecting the downloaded PDF — for example by storing it securely, not sharing it inappropriately, and deleting copies you no longer need.
Future changes to processing
If cloud storage, user accounts, email delivery, practitioner dashboards, PDF hosting, analytics or backend processing are added in future versions, the platform will require expanded privacy controls, a lawful basis assessment, security review and appropriate data protection documentation (including, where applicable, a full privacy notice under UK/EU GDPR or equivalent frameworks) before being deployed to the public.
Emergencies
PERHORC does not provide emergency care. For urgent symptoms such as chest pain, fainting, severe breathlessness, symptoms of stroke, severe allergic reaction or suicidal thoughts, contact your local emergency service immediately (for example 999 or NHS 111 in the UK).
How to clear locally stored data
You can clear the questionnaire data this website stores in your browser at any time. The simplest options:
- Open the questionnaire page and use the Clear My Local Data button.
- Or, in your browser, open Settings.
- Go to Privacy or Site Data settings.
- Search for
perhorc.com(or the current site domain). - Delete stored site data / cookies / local storage for that site.
- Refresh the website.
Exact wording varies between Chrome, Safari, Firefox, Edge and mobile browsers, but every major browser exposes a way to clear stored site data for a specific website.
