1. Purpose of the platform
Use of PERHORC is for general informational, educational and wellness-planning purposes only. It is designed to help you organise self-reported information for later discussion with a qualified professional.
2. Not a substitute for professional advice
The platform does not provide medical advice, diagnosis, prescription, treatment, emergency care or professional judgement. You must seek appropriate professional review — from a GP, registered dietitian, physiotherapist, certified fitness professional, consultant physician, mental-health professional or other appropriately qualified practitioner — before implementing any strategy generated by the platform.
3. Accuracy of information you provide
You are responsible for the accuracy, completeness and honesty of the information you enter into the questionnaire. The platform may produce incomplete, misleading or unsuitable outputs if you provide incomplete, hurried, inaccurate or misleading answers.
4. Age restriction (18+)
PERHORC is intended for adults aged 18 and over. It is not designed for children or adolescents and should not be used for paediatric weight, diet, fitness or health planning unless reviewed and adapted by appropriately qualified professionals.
5. Not for emergencies
PERHORC must not be used in place of emergency services. For chest pain, fainting, severe breathlessness, symptoms of stroke, severe allergic reaction, suicidal thoughts or any other urgent medical concern, contact your local emergency service (for example 999 or NHS 111 in the UK) immediately.
6. Confidentiality of the downloaded report
The downloaded PDF may contain sensitive personal information. You should treat the downloaded PDF as confidential and take reasonable steps to protect it (secure storage, controlled sharing, deletion of unused copies).
7. Limitation of liability
To the fullest extent permitted by applicable law, the website operators, creators, owners, developers, partners and affiliates accept no liability whatsoever for any injury, illness, loss, damage, adverse outcome, misuse, misinterpretation or reliance arising from use of the platform, the generated report or any information provided by the platform.
8. Changes to the platform
The platform is a prototype and may be updated, modified, restricted or withdrawn at any time without notice. Features, content, disclaimers and versioning may change between releases.

By using PERHORC, submitting the questionnaire or downloading the generated report, you confirm that you have read, understood and accepted these Terms of Use.
